Does statins break a fast?

No. Statin tablets have no calories and do not break a fast. Simvastatin and atorvastatin can be taken with or without food.

A statin tablet provides no calories, sugar or protein. It does not break a fast for weight loss, autophagy, gut rest or blood sugar, the four goals this site uses. Statins lower cholesterol and help prevent heart attacks and strokes.

Simvastatin and atorvastatin can both be taken with or without food, so a dose with water during the fast keeps you fasted. Simvastatin is usually taken in the evening, and its US label says once daily in the evening. Atorvastatin can be taken at any time of day. Whatever your statin, take it at the time your prescriber set. Statins are usually taken for a long time, sometimes for life, so the timing has to work on every day of your fasting schedule.

Take care with what you break the fast with. Grapefruit juice can increase the side effects of some statins. With simvastatin, avoid grapefruit juice. With atorvastatin, have no more than 1 or 2 small glasses a day. Keep alcohol down as well, as it adds to the risk of side effects. Common side effects of statins include headaches, feeling sick, and constipation or diarrhoea.

Some antibiotics, antifungal medicines and the herbal remedy St John's wort can interact with statins. Mention your statin whenever you are prescribed something new. Tell your doctor if you are pregnant or planning a pregnancy.

Never skip, delay or stop a statin to fit a fasting schedule. If you take another statin, check its leaflet for food and timing advice, or ask a pharmacist. If you get unexplained muscle pain, weakness or cramps, contact a doctor or pharmacist straight away.

Watch out

Grapefruit juice is a poor choice to break a fast if you take simvastatin or atorvastatin. It can interfere with these statins and increase side effects.

Questions people ask

Can I take my statin during my fasting window?

Yes. Simvastatin and atorvastatin can be taken with or without food, so a dose with water keeps you fasted. Keep to the time your prescriber set.

Does intermittent fasting lower cholesterol?

A 2025 review pooled 99 trials. Intermittent fasting and ordinary daily calorie cutting had similar effects on weight and on heart and metabolic risk factors. That is moderate evidence. Fasting does not replace a statin, so keep taking yours unless your doctor changes it.

Medication safety: never stop, delay or change a prescribed medicine to protect a fast. If a medicine needs food, take it with food. Speak to your prescriber or pharmacist about timing. See our medical disclaimer.
